Website Intake Form

Thinking about a website — for a business, a project, a cause, an event, a campaign, or just an idea you want out in the world? Fill this out in your own words — the more real detail you give here, the closer the first draft lands to the site you have in your head. There are no wrong answers, and “not sure yet” is a perfectly good one.

How this works:

  • Seven short sections — about 10–15 minutes. You can move back and forth freely.
  • At the end you’ll see a clean summary of everything you entered — one tap on Send to Allen sends it straight to Allen, and that summary is what gets your site started. You can also download a copy for your own records.
  • Fields marked * are the only required ones.

Who you’ll be working with

We’re local — Allen & Daniela Clark, based in Cavendish, Vermont. Real people right here in Okemo Valley, not an outside company.

Allen Clark — design, build & upkeep

Allen designs and builds your website — the look, the pages, the words and photos — and he’s the one who maintains it and keeps it updated after it launches. He’s local, in Cavendish, and he’s the person you’ll actually talk to.

Daniela Clark — professional web expertise

Allen’s wife Daniela is a professional Web Manager for a large international company in the boating industry. She brings that professional, technical expertise to the team — a seasoned second set of eyes on how a serious website should work.

Websites we’ve built — for reference

We created all of these websites ourselves. We design and build them — the look, the pages, the words and photos — and we maintain them and keep them updated. Take a look through them for a sense of what we build and the different kinds of sites we can make for you.

1 · The basicsStep 1 of 7

1 · The basics

Start with who you are and what this website is for. A business fits here — but so does a project, a cause, an event, or a campaign. Skip anything that doesn’t apply.

Where Allen should reply — this doesn’t have to be the email shown on the site.
Please fill in your name, the name of the website or project, your email, and a line or two about what this website is for — Allen needs those four to get started.

2 · What the site should do

A website is a tool. Tell us the job it needs to do, and who it’s doing it for.

The main job of the site

Pick everything that applies — then star the big one in the box below.

3 · Your ideas & style

This is the “site in your mind” section. Describe it however it comes out — examples help more than fancy words.

The feel of the site

Pick up to a handful that fit.

4 · Pages & features

Tick what you think you need. Not sure? Tick it anyway and note it — we’ll sort out the final list together.

5 · Content you have

For each item: do you already have it, or do you need help creating it? “Need help” is a normal answer — just say so here instead of stalling the project later.

LogoA file, a photo of your sign, or nothing yet — all workable.
PhotosThe place, the people, the work, the cause — whatever tells the story.
Words / text for the pagesDescriptions, your story, service details.
Price list, menu, or info sheetCurrent version, even if it’s a photo of a printed one. Skip if it doesn’t apply.
Reviews, kind words, or endorsementsFrom customers, neighbors, supporters — Google reviews count too.
Please don’t type passwords into this form — not for your domain, email, Facebook, or anything else. If logins are needed, Allen will collect them securely with you directly, later.

6 · What you have now, and timing

Your current online footprint and your calendar — this is what decides how the build is sequenced.

Keeping the site updated afterwards

7 · Review & send it to Allen

Here’s your intake, gathered into one clean summary. Check it over — you can jump back to any section to fix something — then tap Send to Allen. One tap sends it straight to Allen through this website; you don’t have to copy, paste, or email anything yourself.

The download is just a copy for your own records. If a send ever doesn’t go through — no connection, for example — keep that downloaded copy so your answers aren’t lost, and let Allen know.
